A service is available on this site to allow you to submit fill out and print your INSTANT MEDICAL HISTORY. This could save valuable time during your office visit. This service is for new patients or established patients coming in for NEW problems. To access this service, click above under HOME PAGE/Practice Members and "CLICK FOR TRI-COUNTY PAIN MANAGEMENT", look at the Patient Login Box and look down under SHORTCUTS. Click on COMPLETE AN iHEALTH INTERVIEW, set up a free account, fill out the forms, print them, bring them to your next visit. | | ONLINE REGISTRATION
For your first visit to our office, valuable time can be saved by providing us with the necessary information for registration in advance. This form will be submitted to us by secure email. This form may be filled out ONLINE.
Established patients may also use this form to update any information which may have changed since your last visit.
